CCM HEALTHCARE ASSISTANCE FUND FOR RETIRED MINISTERS
中信溫哥華中心 退休牧者醫療資助基金

This fund provides subsidies for medical services to eligible retired pastors or missionaries.
此基金為合資格的退休牧者或宣教士提供醫療服務上的資助。

Terms and Conditions

WHO can apply:

  • Must be age 65 or older.
  • Pastors or missionaries who have ministered in a church or mission organization for a minimum of 4 years before retirement.
  • Currently not employed by a church or organization.
  • A permanent resident of B.C.

WHAT is the funding scope:

  • Funding covers the applicants and their spouses only.
  • Up to a maximum of CAD$1,000 per applicant or $1,500 per couple per fiscal year.
  • Subsidy coverage includes dental, eye, ear, medicine and other medical expenses.
    Note: Only apply to medicine that is prescribed by your physician.

HOW to apply:

  • Fill out application form:
    • Method 1 : Fill out the form online and upload all the relevant receipts; or
    • Method 2 : Submit the form with the relevant receipts in-person to CCM Vancouver Centre Burnaby Office (4533 Kingsborough St., Burnaby – 2/F by the food court). Please submit all the above information in a sealed envelope and attention to “Mable Law.”
  • The CCM HAF Representative will contact the applicant to learn more about the status of the application.
  • Once application is approved, a cheque will be issued and mailed to applicant by end of January, 2025.

WHEN to apply:

  • Application begins November 18, 2024 until December 16, 2024
  • When submitting the application form, please attach valid medical invoices dated between December 1, 2023 and November 30, 2024 that have not been claimed from any sources before.

  • 資助範圍包括牙齒 、眼睛及耳朵和藥物等醫療費用 。
    注意：只適用於醫生處方的醫療用品和藥物。

申請流程：

  • 填寫申請表格：
    • 方法一：網上直接完成填寫申請表和上載相關的收據；
    • 方法二：到中信溫哥華中心本比辦事處（麗晶廣場二樓美食廣場則，4533 Kingsborough St., Burnaby, BC, V5H 4V3）遞交已填妥之申請表和有關的收據，請把以上資料放進密封的信封內，並註明收件人為：羅利美寶師母（Mable Law）。
  • 有關部門約見了解有關申請狀況。
  • 一旦申請獲得批准, 支票將於2025年1月底前郵寄給申請人。

申請日期：

  • 申請日期：2024年 11月18日至12月16日。
  • 提交申請表時請附上由2023年12月1日至2024年11月30日為止的有效醫療發票（請僅提交未由保險計劃報銷或未從其它來源索取的金額）。
